Disability Benefits Questionnaires (DBQs) are VA forms for doctors to complete to document a veteran's disabilities and their severity.
With Disability Benefits Questionnaires (DBQs), veterans have the option of visiting a private health care provider instead of a VA facility to complete their disability evaluation form. However, please note that the VA will not pay any costs for the completion of DBQs. Also, in some instances, the VA may dispute a DBQ if it is not completed correctly, accurately or legibly. That is why veterans should have their representatives review the DBQs before sending them to the VA.
